Joined in Care Cardiac Program
Our Joined in Care Cardiac Program is a care program designed to treat people with advanced-stage cardiac disease. The program offers a personalized plan of care to provide physical, emotional and spiritual comfort based on the unique needs of each individual patient. Our team of skilled, compassionate professionals take care of the needs of the patient as well as family members or friends caring for the patient.
Heart disease does not come on suddenly. We understand you have struggling with this for a long time. The Joined in Care Cardiac Program hospice nurse will build a relationship with you for the first encounter. She will listen to you, get to know you and come to understand what having heart disease means to you. Through getting to know you and your story, learning what having heart disease has been like for you and how it has been different in the last few months, the nurse can develop a personalized care plan to meet your specific needs.
Comfort and Relief from Symptoms
Through our Joined in Care Cardiac Program, our team of skilled, compassionate professionals takes care of the needs of the patient as well as family members or friends caring for the patient. We respond rapidly to ensure comfort and relief of symptoms.
When to call Hope Hospice & Palliative Care Rhode Island:
- Increased breathlessness
- Increase in fatigue
- Weight gain of greater than two pounds in one day
- Difficulty breathing during the night
- Pain in the chest, arms, jaw or back not relieved with the current medications.
